With the first legal action following the doubly fatal May 1 fire (see our June 11 post) come the first reliable reports about the possible causes:A blast at the clinic dislodged a tube attached to a hyperbaric chamber, resulting in an explosion and flash fire, authorities said. How that happened is still under investigation.
